Initial Phone Consultation

To determine whether I can help you, I would like to offer you a 15 minute free phone consultation. You can call me at (703) 479-5610. If I can’t answer the phone, I will return your call by the end of the next business day. Please indicate the best number to reach you at and a couple of good times to reach you, when you will have the time and privacy to consult with me on the phone. You can also email me this information, please use the contact page for this.


The First Appointment and beyond

Prior to your first appointment, download and complete all the forms. The nature and length of the first appointment will depend on the service you request.

Neuropsychological Assessment

The first appointment will be a clinical interview lasting up to 90min. It is very helpful if you can come to this appointment with the family member who is closest to you and most involved in your care. We will then set up an extended appointment for testing (usually 3-4hrs) Finally, we will schedule a follow up appointment in which I will give you the written report, explain your results, and provide treatment recommendations. I understand that you want to know your results and start treatment, if needed, as soon as possible. I will therefore provide your report in a week's time.

If you prefer to schedule the initial interview and testing on the same day, we can make special arrangements to that end.

What to bring and send
Bring all the forms. Prior to your first appointment please fax your medical records. These include at a minimum:
  • Your most recent History and Physical Exam from your physician
  • Your History and Physical Exam from your last hospitalization related to the reason you are seeking evaluation
  • Your medical discharge summary from the hospital
  • Your discharge summaries from therapies (OT, PT, SLP) if applicable
  • A list of your current medication and dosage (you can use my form if you prefer)
Important: I will not be able to proceed with your appointment if I do not receive your medical records


Cognitive Rehabilitation

In your first appointment we will identify your desired scope of treatment. It is very helpful if you can come to this appointment with the family member who is closest to you and most involved in your care. This appointment will last up to 75 minutes.

What to bring/send
Bring all the forms. Prior to your first appointment please fax your medical records. These include at a minimum:
  • Your most recent History and Physical Exam from your physician
  • Your History and Physical Exam from your last hospitalization related to the reason you are seeking evaluation
  • Your medical discharge summary from the hospital
  • Your discharge summaries from therapies (OT, PT, SLP) if applicable
  • A list of your current medication and dosage (you can use my form if you prefer)

Therapy

During the initial appointment we will create a framework for what you want to accomplish and how to proceed. If we find that I will not be able to help you or you don’t feel it’s a good fit for any reason, I will do my best to provide you with a good referral.

This initial assessment is longer, typically around 75minutes.

Insomnia

This initial evaluation is aimed at differential diagnosis to determine whether you have insomnia and whether CBT-I is indicated for your presentation. If so, I will explain the rationale for treatment and start setting up the treatment itself. I also assess for psychological comorbidities and discuss with you how these might be addressed. This requires time: typically around 90 minutes.

What to bring
Bring all the forms:.
  • If you have been referred by a physician, bring the most recent exam
  • If you have been seen by a sleep specialist, typically a pulmonologist or a neurologist, bring the reports they gave you: the written report of the physician’s visit and reports of any testing procedure such as polysomnography (i.e. the sleep study), actigraphy (the bracelet), etc...

Rapid Resolution Therapy

The first appointment is an initial evaluation lasting up to 90 minutes. We will determine if RRT is right for you and create your accelerated treatment plan.
